KANSAS CITY SECRET FOR WEIGHT LOSS – PROGRAM AGREEMENT

Congratulations on your investment in our weight loss program! This agreement will outline the guidelines and protocols of successfully completing the program and maintaining your results.
I agree to enter into the weight loss program willingly and with the highest expectation of success following the guidelines outlined in our Program Guide. I desire to lose weight and take control of my health and well-being.
Through the Kansas City Secret for Weight Loss Secret Program you will have access to the proprietary weight loss system known as the Weight Loss Program, 6 month program, 2 month supply of Lipo supplement, eating plan and guidelines, weigh in/follow up appointments, unlimited consults/support, food journal tracking chart, and exclusive customer service and support.
The Weight Loss Program is designed to:
1. Create a proven blueprint to success for weight loss.
2. Establish a Doctor Supervised diet model to maintain predictable results.
3. Enable clients to take control of their weight loss goals for greater success.
4. Provide a lifestyle change for future health benefits and longevity.
5. Increased energy, improved sleep, increased inches loss, better metabolism, and reduced appetite.
To protect the integrity of the brand and the program, you agree to:
1. Clearly define your goals and the result you would like from the program.
2. Follow the steps to a successful “before/after photo.”
3. Agree to and schedule the required follow up appointments for accountability (virtual or in-person).
4. Follow the Weight Loss Program guidelines and protocols exactly as outlined.
5. Keep accurate records of your daily meals and the Weight Loss Program Lipotropic formula dosages.
6. We do use email as a tool to communicate during your Weight Loss Journey, unless you opt out of it.
- I agree and understand that my supervising doctor and health coaches will review my progress and willingness to successfully complete the program. If at any time, it is determined that I am neither willing nor desire to continue with the program, I understand that there may be alternative programs that may be available. After starting the program, I understand that I am responsible for my success and no refund will be offered by the doctor or the Kansas City Secret for Weight Loss program. I also understand that there are no guarantees with any program regarding your health, especially when it comes to weight loss.
- “Physician Supervised” – The medical team at Kansas City Secret for Weight Loss maintains a direct line of communication with all staff and health coaches regarding your weight loss journey. We will not adjust medications nor prescribe medications – that needs to be handled by your prescribing physician. If you would like to speak with one of our medical providers, please request through our text line or ask a health coach and one will reach out as soon as they can.
- It is the client’s responsibility to schedule check-in/follow-up appointments.
If you run out of Lipo drops early there will be a charge ($196), use as directed on each dose. Lipo Drops will last a minimum of 60 days if taken as recommended. NOT 6 MONTHS.
The goal is to NOT have you taking supplements forever, but to teach you how to lose and maintain weight on your own – thus never needing our, or another weight loss program again.
This is a 6-month commitment. We are here for you on this journey and will do everything we can to assist you – but ultimately it is your responsibility to check in with us and ask for help.
